Insurance Coverage for Electrolysis: A Step-by-Step Guide

Many health plans cover electrolysis when it is medically necessary, for example for gender-affirming care, hair removal before gender-affirming surgery, or hirsutism caused by conditions such as PCOS. Coverage varies by plan. Celestial Electrolysis cannot guarantee coverage, but we provide the documentation you need to ask for it.

How we work with insurance

We are not contracted (in network) with any insurance plan. When your plan gives you a prior authorization for electrolysis with Celestial Electrolysis, we bill your insurance directly. This applies to Kaiser Permanente and other carriers, but not to San Francisco Health Plan (see below).

Without a prior authorization, we can provide superbills, receipts, and treatment records that you can submit to your plan yourself.

Step 1: Contact your insurance provider

Call the Member Services number on your insurance card and ask about:

  • Coverage for medically necessary electrolysis
  • Prior authorization requirements
  • Out-of-network benefits
  • A Single Case Agreement (SCA), Letter of Agreement (LOA), or network gap exception

You can say: "I am seeking medically necessary electrolysis services and would like to request authorization to receive care from my chosen provider, Celestial Electrolysis."

Electrolysis is usually billed with procedure code CPT 17380 (electrolysis epilation, each 30 minutes). Your doctor provides the diagnosis code.

Step 2: Get supporting medical documentation

Your plan may ask for documentation from your healthcare provider, such as:

  • A letter of medical necessity
  • A treatment recommendation
  • Relevant diagnosis documentation
  • Documentation from your surgeon, for hair removal before surgery

Step 3: Request a Single Case Agreement or Letter of Agreement

If your plan approves care with Celestial Electrolysis, it may issue a written agreement. Before you start, confirm that it includes:

  • Celestial Electrolysis as the approved provider
  • Approved dates of service
  • Approved number of units or hours
  • Reimbursement terms
  • Any required billing steps

Step 4: Billing and reimbursement

With an approved prior authorization, we bill your plan directly. If your plan asks you to submit claims yourself, or you do not have an authorization, we can provide:

  • Superbills
  • Receipts
  • Treatment records

Notes for common plans

These notes are general. Your plan's own rules always apply.

Kaiser Permanente. Kaiser covers gender-affirming facial hair removal for members when a Kaiser clinician confirms medical necessity. Celestial Electrolysis is not contracted with Kaiser. Ask your Kaiser gender-affirming care team for a prior authorization for care with Celestial Electrolysis; once it is approved, we bill Kaiser directly.

Medi-Cal and San Francisco Health Plan. Some Medi-Cal plans, including San Francisco Health Plan, cover gender-affirming hair removal with a prior authorization from your primary care provider. Celestial Electrolysis is not an authorized San Francisco Health Plan provider, so SFHP members should ask their primary care clinic for the plan's authorized hair removal providers.

Blue Shield, Aetna, Anthem, and other commercial plans. Many commercial plans cover gender-affirming hair removal and surgical-prep hair removal with prior authorization. Some have a specific authorization form; ask Member Services for it. Once your plan approves care with Celestial Electrolysis, we bill your plan directly.

Hirsutism and PCOS. Coverage for hirsutism is less common, but some plans approve it with documentation from your doctor.

HSA and FSA. Medically necessary electrolysis may be eligible. Check with your plan administrator; a letter of medical necessity is often required.

Important information

Insurance policies and benefits vary. Approval, reimbursement amounts, and eligibility are decided by your plan and are not guaranteed by Celestial Electrolysis. We encourage you to talk directly with your insurance provider about coverage, benefits, and reimbursement. Our goal is to support you with the documentation you need to move through the process with confidence.
