Hair Removal Before Gender-Affirming Surgery

Many surgeons require permanent hair removal before vaginoplasty, phalloplasty, and metoidioplasty, so that hair does not grow inside the new anatomy after surgery. Electrolysis permanently removes hair of every color, including the light and grey hairs that laser misses. Jade at Celestial Electrolysis in San Francisco has years of experience preparing clients for gender-affirming surgery.

Why surgeons require hair removal

During some procedures, skin from the genital area, forearm, or thigh is used to build new tissue, such as the vaginal canal or the urethra. If hair follicles stay in that skin, hair can grow where it causes irritation, infection, or blockage. Removing the hair before surgery prevents this.

Which areas need treatment

Your surgeon decides the exact area. Most surgical teams give you a hair removal map (a diagram or template) that shows which skin must be clear. Bring it to your first appointment.

  • Vaginoplasty: usually parts of the scrotum and the shaft of the penis, depending on your surgeon's technique.
  • Phalloplasty: the donor site, often the forearm or the thigh, and sometimes other areas your surgeon names.
  • Metoidioplasty: sometimes an area used to lengthen the urethra, if your surgeon includes it.

How long it takes

  • Start early. Surgeons commonly ask for 6–12 months or more of hair removal before surgery, because follicles grow in cycles and each one must be treated when it is active.
  • Sessions every few weeks. The same area needs time to heal between sessions. We plan a schedule around your surgery date.
  • A clear final check. Many surgeons want the last session a few weeks before surgery so the skin is fully healed. Follow your surgeon's exact instructions.

Electrolysis or laser?

Laser can reduce dark hair quickly, and some clients start with laser. But laser does not work on light, grey, white, or red hair, and some hairs survive it. Many surgeons accept only electrolysis, or require electrolysis to finish the area. We can work with you whether you use electrolysis alone or after laser.

Comfort and privacy

Genital electrolysis is done in a private room with care and respect. Many clients use a numbing cream prescribed by their doctor. We explain each step before we start, and you can take breaks at any time.

Insurance

Hair removal before gender-affirming surgery is often covered by insurance when your surgery is approved. Your plan may require a letter from your surgeon and a prior authorization. With a prior authorization, we bill your insurance directly. See our insurance guide.

Questions we often hear

Do I need electrolysis before bottom surgery? It depends on your surgeon and your procedure. Many surgeons require hair removal for vaginoplasty and phalloplasty. Ask your surgeon for their hair map.

How many hours of electrolysis does bottom surgery prep take? It depends on the size of the area and your hair density. Many clients need dozens of hours over 6–12 months or more. We give you an estimate at your first visit.

When should I start? As soon as you have a likely surgery date, or earlier. Starting 12 months before surgery gives the most room.

When should my last session be? Follow your surgeon's instructions. Many surgeons ask for the last session a few weeks before surgery.
